Homecare Agency maintains continuum of care for all services provided to all Homecare Agency patients. This will be accomplished through the utilization of on-call nursing and administrative personnel.

 

 

1)                  A RN will be available to the patient/family on a 24 hour basis. The RN will have access to a supervisor/manager on a 24 hour basis.

 

2)                  The following personnel may be on-call during non-business times:

a)      Administrator

b)      Director of Patient Care Services

c)      RN/Nursing Supervisor

d)      RN/LPN (Field, case manager, Specialty Nurses)

e)      Scheduler/Coordinator

 

3)                  All on-call personnel will maintain the following:

a)      Current active patient list which must include name, address, phone number, emergency contact and all active services for each patient.

b)      Current employee list to include name address, phone number, pager and discipline.

c)      All pertinent administrative numbers.

d)      The agency’s emergency preparedness plan

 

4)                  When the Homecare Agency provides Aide Services after normal working hours, the organization must be able to provide the services as necessary to meet client/patient needs and in accordance with the client/patient’s plan of care. A scheduler must be on-call to provide access to services after office hours.

 

5)                  All communication will be through telephone and paging systems.

 

6)                  All on-call personnel must respond to pages within fifteen (15) minutes.

 

7)                  If it is determined by the on call nurse that a patient is a suicide threat, homicide threat, experiencing anxiety or panic attack, or is in a life threatening situation, the on call nurse will ensure that the police, crisis services or appropriate area 911 is called.
